This article presents a unique clinical case of a female patient with an exceptionally complex form of hypertrophic cardiomyopathy, combined with a subaortic muscular membrane and a fixed-dynamic obstruction of the left ventricular outflow tract. The patient had multiple atrial arrhythmias (atrial fibrillation and flutter), underwent repeated catheter procedures (extended transcatheter substrate modifications for arrhythmia), and developed complications leading to stiff left atrium syndrome with atrial wall calcification and severe pulmonary hypertension with pulmonary artery aneurysm. The necessity of heart–lung transplantation is substantiated. The material has educational and practical value for specialists.
